Whether it is or isn't your first time visiting a therapy clinic, we know there will be a lot of questions and you may feel uncertain. Here are some things you can do before hand to be prepared to meet us for the first time:

Paperwork

If you can, fill out the paperwork before your first visit or eval. We can email this to you ahead of time, or you can find it on our website under our 'Getting Started' section, but if you are unable to print it out, make sure you arrive 30 minutes early so you have plenty of time to fill it out. We will be available during and after your appointment to answer any questions you have about this!

You will need to fill out the following:

  • Child History
  • New Patient Packet
  • Physical Therapy, Occupational Therapy, Speech Language or Feeding Therapy History form (based on what you are visiting us for)

Prepare Questions

Before you even arrive, it may be helpful to jot down all the questions that you have for us. We know there can be a lot to discuss at a first appointment and it's easy to leave things out - a list makes this easier! If it's incredibly long, you can even leave it with your therapist so they can think through their answers and get back to you ASAP.

Bring Toys

The office and therapy room will be a completely new place to your child, and they may feel uncomfortable with the unfamiliar feeling. We have plenty of toys and fun things, but it can also be helpful to bring along something from home that they love - we encourage it!
